Problem China continues to face challenges in eliminating mother-to-child transmission of human immunodeficiency virus (HIV), syphilis and hepatitis B virus (HBV). Approach In 2010, a programme that integrated and standardized prevention of mother-to-child transmission (PMTCT) efforts for HIV, syphilis and HBV was implemented in 1156 counties. At participating antenatal care clinics, pregnant women were offered all three tests concurrently and free of charge. Further interventions such as free treatment, prophylaxis and testing for mothers and their children were provided for HIV and syphilis. Local setting China’s national PMTCT HIV programme started in 2003, at which time there were no national programmes for perinatal syphilis and HBV. In 2009, the rate of maternal-to-child transmission of HIV was 8.1% (57/702). Reported congenital syphilis was 60.8 per 100 000 live births. HBV infection was 7.2% of the overall population infected. Relevant changes Between 2010 and 2013 the number of pregnant women attending antenatal care clinics with integrated PMTCT services increased from 5.5 million to 13.1 million. In 2013, 12.7 million pregnant women were tested for HIV, 12.6 million for syphilis and 12.7 million for HBV. Mother-to-child transmission of HIV fell to 6.7% in 2013. Data on syphilis transmission are not yet available. Lessons learnt Integrated PMTCT services proved to be feasible and effective, and they are now part of the routine maternal and child health services provided to infected women. The services are provided through a collaboration between maternal and child health clinics, the national and local Centers for Disease Control and Prevention, and general hospitals.

Resumen Situación China sigue haciendo frente a desafíos en la eliminación de la transmisión maternoinfantil del virus de la inmunodeficiencia humana (VIH), la sífilis y el virus de la hepatitis B (VHB). Enfoque En 2010 se puso en marcha en 1156 condados un programa que integraba y estandarizaba los esfuerzos de prevención de la transmisión maternoinfantil (PTMI) para el VIH, la sífilis y el VHB. En las clínicas de atención prenatal que participaron se ofrecieron a las mujeres embarazadas las tres pruebas al mismo tiempo y de forma gratuita. El programa incluyó otras intervenciones como el tratamiento gratuito, profilaxis y pruebas para las madres y sus hijos. Marco regional El programa nacional de PTMI del VIH en China comenzó en 2003, momento en el que no había ningún programa nacional para la sífilis y el VHB perinatales. Objective To assess medical and nursing students’ intentions to migrate abroad or practice in rural areas. Methods We surveyed 3199 first- and final-year medical and nursing students at 16 premier government institutions in Bangladesh, Ethiopia, India, Kenya, Malawi, Nepal, the United Republic of Tanzania and Zambia. The survey contained questions to identify factors that could predict students’ intentions to migrate. Primary outcomes were the likelihoods of migrating to work abroad or working in rural areas in the country of training within five years post-training. We assessed predictors of migration intentions using multivariable proportional odds models. Findings Among respondents, 28% (870/3156) expected to migrate abroad, while only 18% (575/3158) anticipated a rural career. More nursing than medical students desired professions abroad (odds ratio, OR: 1.76; 95% confidence interval, CI: 1.25–2.48). Career desires before matriculation correlated with current intentions for international (OR: 4.49; 95% CI: 3.21–6.29) and rural (OR: 4.84; 95% CI: 3.52–6.66) careers. Time spent in rural areas before matriculation predicted the preference for a rural career (20 versus 0 years: OR: 1.53, 95% CI: 1.19–1.98) and against work abroad (20 versus 0 years: OR: 0.69, 95% CI: 0.50–0.96). Conclusion A significant proportion of students surveyed still intend to work abroad or in cities after training. These intentions could be identified even before matriculation. Admissions standards that account for years spent in rural areas could promote greater graduate retention in the country of training and in rural areas.

Resumen Objetivo Evaluar las intenciones de los estudiantes de medicina y enfermería de emigrar al extranjero o trabajar en zonas rurales. Métodos Se encuestó a 3199 estudiantes de medicina y enfermería de primer y último curso de 16 instituciones gubernamentales de primer nivel en Bangladesh, Etiopía, India, Kenya, Malawi, Nepal, Tanzania y Zambia. La encuesta incluyó preguntas para identificar los factores que podrían predecir las intenciones de emigrar de los estudiantes. Los resultados primarios fueron las probabilidades de emigrar para trabajar en el extranjero o en zonas rurales en el país de formación en los cinco años posteriores a esta. Los indicadores de las intenciones de migración se evaluaron por medio de modelos de probabilidades proporcionales multivariables. Problem Despite seven years of investment from the President's Emergency Plan For AIDS Relief (PEPFAR), the expansion of human immunodeficiency virus (HIV)-related services continues to challenge Mozambique’s health-care infrastructure, especially in the country’s rural regions. Approach In 2012, as part of a national acceleration plan for HIV care and treatment, Namacurra district employed a mobile clinic strategy to provide temporary manpower and physical space to expand services at four rural peripheral clinics. This paper describes the strategy deployed, the uptake of services and the key lessons learnt in the first 18 months of implementation. Local setting In 2012, Namacurra´s adult population was estimated to be 125 425, and of those 15 803 were estimated to be HIV infected. Although there is consistent government support of antiretroviral therapy (ART) programmes, national coverage remains low, with less than 15% of those eligible having received ART by December 2012. Relevant changes Between April 2012 and September 2013, Namacurra district enrolled 4832 new patients into HIV care and treatment. By using the mobile clinic strategy for ART expansion, the district was able to expand provision of ART from two to six (of a desired seven) clinics by September 2013. Lessons learnt Mobile clinic strategies could rapidly expand HIV care and treatment in under-funded settings in ways that both build local capacity and are sustainable for local health systems. The clinics best serve as a transition to improved capacity at fixed-site services.

Resumen Situación A pesar de los siete años de inversión del Plan de Emergencia del Presidente de los Estados Unidos de América para luchar contra el SIDA (PEPFAR), la expansión de los servicios relacionados con el virus de la inmunodeficiencia humana (VIH) continúa suponiendo un desafío para la infraestructura sanitaria de Mozambique, especialmente en las regiones rurales del país. Enfoque En 2012, como parte de un plan de aceleración nacional para el cuidado y tratamiento del VIH, el distrito de Namacurra empleó una estrategia clínica móvil para suministrar mano de obra y espacio físico temporales para expandir los servicios a cuatro clínicas periféricas rurales. Este documento describe la estrategia empleada, la comprensión de servicios y las lecciones clave aprendidas en los primeros 18 meses de implementación. Marco regional En 2012, la población adulta estimada de Namacurra era de 125 425 y de estas personas se estimaba que 15 803 estaban infectadas con el VIH. Mediante el uso de la estrategia de la clínica móvil para la expansión de TAR, el distrito pudo expandir las provisiones de TAR de dos a seis clínicas (de las siete deseadas) antes de septiembre de 2013. Lecciones aprendidas Las estrategias de las clínicas móviles podrían expandir rápidamente el cuidado y tratamiento del VIH en situaciones de poca financiación de forma que crean capacidad local y sean sostenibles para los sistemas sanitarios locales. Las clínicas sirven de la mejor manera como transición a la capacidad mejorada en servicios en lugares fijos.

OBJECTIVES: To describe the frequency of HTLV-1 infection among offspring of mothers who had presented with HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), strongyloidiasis, or asymptomatic HTLV-1 infection, and to identify factors associated with HTLV-1 infection. METHODS: In a descriptive study, records were reviewed of HTLV-1-positive women and their offspring who had been tested for HTLV infection at a public hospital in Lima, Peru, from 1989 to 2003. Sons and daughters of women who had presented with strongyloidiasis, HAM/TSP, or asymptomatic infection were eligible for this study. RESULTS: Three hundred seventy subjects were included: 279 were the offspring of 104 mothers presenting with HAM/TSP, 58 were the offspring of 22 mothers with strongyloidiasis, and 33 were the offspring of 26 asymptomatic mothers. Mean age of the offspring at the time of testing was 26 years (standard deviation 12). Nineteen percent of the offspring tested positive for HTLV-1: 6% (2/33) of those with asymptomatic mothers, 19% (52/279) among the offspring of mothers with HAM/TSP, and 31% (18/58) among the offspring of mothers presenting with strongyloidiasis On multiple logistic regression analysis, three factors were significantly associated with HTLV-1: (a) duration of breast-feeding (odds ratio [OR] = 15.1; [4.2-54.1] for 12 to 24 months versus less than 6 months breast-feeding); (b) clinical condition of the mother (OR = 8.3 [1.0-65.3] for HAM/TSP and OR = 11.5 [1.4-98.4] for strongyloidiasis in comparison with offspring of asymptomatic mothers); and (c) transfusion history (OR = 5.5 [2.0-15.2]). CONCLUSIONS: In addition to known risk factors for HTLV-1 transmission (duration of breast-feeding and history of blood transfusion), maternal HAM/TSP and strongyloidiasis were associated with seropositivity among offspring of HTLV-1-infected mothers.

OBJETIVOS: Describir la frecuencia de la infección por HTLV-1 en los hijos e hijas de madres diagnosticadas con mielopatía/paraparesia espástica tropical asociada con el HTLV-1 (M/PET-HTLV-1), estrongiloidiasis o infección asintomática por HTLV-1, e identificar los factores asociados con la infección por HTLV-1. MÉTODOS: Para este estudio descriptivo se revisaron los registros de mujeres positivas a HTLV-1 y de sus hijos evaluados con pruebas para la infección por HTLV en un hospital público de Lima, Perú, entre 1989 y 2003. Eran elegibles para este estudio los hijos y las hijas de las mujeres que se presentaron con estrongiloidiasis, M/PET-HTLV-1 o infección asintomática. RESULTADOS: En el estudio participaron 370 personas: 279 hijos de 104 madres con M/PET-HTLV-1, 58 hijos de 22 madres con estrongiloidiasis y 33 hijos de 26 madres asintomáticas. La edad promedio de los participantes en el momento de su prueba para HTLV era de 26 años (desviación estándar: 12 años). De las personas estudiadas, 19% resultaron positivas a la infección por HTLV-1: 6% (2/33) de los hijos de madres asintomáticas, 19% (52/279) de los hijos de madres con M/PET-HTLV-1 y 31% (18/58) de los hijos de madres con estrongiloidiasis. Según el análisis de regresión logística múltiple, tres factores se asociaron significativamente con la infección por HTLV-1: a) duración de la lactancia materna por 12_24 meses (razón de posibilidades [odds ratio, OR] = 15,1; intervalo de confianza de 95% [IC95%]: 4,2 a 54,1, frente a la lactancia materna por menos de 6 meses); b) que la madre presentara M/PET-HTLV-1 o estrongiloidiasis (OR = 8,3; IC95%: 1,0 a 65,3 y OR = 11,5; IC95%: 1,4 a 98,4, respectivamente, en comparación con los hijos de madres asintomáticas); y c) los antecedentes de haber recibido una transfusión sanguínea (OR = 5,5; IC95%: 2,0 a 15,2). CONCLUSIONES: Además de los factores de riesgo de la transmisión de la infección por HTLV-1 conocidos (duración de la lactancia materna y antecedentes de transfusión sanguínea), el diagnóstico materno de M/PET-HTLV-1 y el de estrongiloidiasis se asociaron significativamente con la infección por HTLV-1 en los hijos de madres seropositivas.

Revista Chilena de Infectología has recently been accepted for index listing in the Index Medicus via the MEDLINE data base. This breakthrough prompts reflection on the nature of regionally focused subspecialty journals and what such a listing means for better dissemination of scientific and medical discoveries reported in such journals. There is a conscientious global effort at present to better represent regional journals in indexing systems and to make access easier for persons in less wealthy nations to medical and public health information via the internet.
